Our aim at Modern Mums Physiotherapy is to deliver a caring, professional and evidence-based Women's Health Service.
Our Terms and Conditions are there to help you understand how we operate our practice.
By commencing treatment you are deemed to have acknowledged and have agreed to be bound by these Terms & Conditions.
This agreement is between the patient (referred to as ‘you’, ‘your’ and “client”) and Modern Mums Physiotherapy (referred to as ‘Modern Mums Physiotherapy’, ‘we’ or ‘us’).
Please read these terms carefully. They provide the basis on which you will be provided goods and services by Modern Mums Physiotherapy.
Payments

Our fees for each session are confirmed by us to you prior to the appointment. Payment for physiotherapy services is due at the time, or on the day of treatment. We accept card payments. Payment by bank transfer (BACS) is available on request. An invoice will be emailed to you following payment to the email address you have provided.

If you are late arriving at a session, the session will begin on your arrival and continue until the time when it is scheduled to end. If you wish the session to overrun, and we agree, then you may be charged at our discretion for the extra time we spend in providing the Services.

If you have booked a session and you cancel with less than 24 hours' notice or do not turn up for the session, then you are liable to pay the cost of that session.

On occasion, we may have to cancel a scheduled session. This may occur, for example, if a staff member is sick or if a preceding session has overrun. If we have to cancel a scheduled session, we will book you another appointment as soon as reasonably possible. No charge will be made to you for the session cancelled by us.

We may stop a session at any time and not continue it if we consider that you are not following our reasonable instructions. If this happens, you will still be liable (at our discretion) to pay the fee for that session.
Consent

Modern Mums Physiotherapy requires the consent of clients both for processing client data and treatment. The client’s electronic signature/tick on the Initial Assessment form and Consent Forms electronically confirms that the client has read and understood the terms and conditions and that client:-

Consents to information including medical details about the client being processed for the purposes of treatments as a private client.

Consents to treatment by the physiotherapist.

Consents to having details, which are relevant to the client’s conditions and for which they are seeking treatment, to be shared with client’s GP/consultant on request.

We make every effort to help you to fully understand our findings, the recommended procedures and any treatment that may be involved. We welcome any queries on anything that you may be uncertain of, or if you simply require more information.

Assessments are carried out in a private, secure environment by a female physiotherapist. You will be asked in detail about your problem and medical history. An internal assessment will only be undertaken with your consent. The pelvic floor muscles can be felt just inside the opening to the vagina. The assessment can tell us how strong or weak the pelvic floor is, or identify any problem areas, such as pain.

We need your permission and signed consent before we can do this examination. It is your right to say “no” if you do not want to be examined, even after you have given consent.
Physiotherapy Assessment

During your initial consultation your Physiotherapist will ask you some questions about your symptoms and will then ask to assess the problem area. Please be aware due to the nature of your condition, you may be asked personal questions relating to your symptoms or problem area. The Physiotherapist will inform you that if you are not happy discussing this subject, it is your right to refuse this discussion.

In order to properly assess you, your Physiotherapist may need to see the affected area. For the assessment, it would be useful if you could bring a pair of shorts to change into, similarly for an upper limb condition bringing a vest top to change in to. If your assessment requires you to undress for a pelvic or breast examination, we recommend wearing clothing that is easily removable and comfortable.

If you feel uncomfortable about the examination, you can bring someone with you or ask us to arrange a chaperone to be present during the examination. Please feel free to ask any questions that you may have, before or at any time during, your session.

After your assessment, your Physiotherapist will discuss their findings with you and propose a treatment plan tailored specifically to your own goals.

This Privacy Policy sets out the basis on which we collect and process personal data about you including our practices regarding the collection, use, storage and disclosure of personal data that we collect from you and/or hold about you, and your rights in relation to that data.

What personal data may we collect from you?

When we refer to personal data in this policy, we mean information that can, or has the potential to identify you as an individual.
Accordingly, we may hold and use personal data about you as a patient or in any other capacity, for example when you complete a form, access our services or speak to us. Depending on what services you receive from us, this may include sensitive personal data such as information relating to your health. We have a legal duty to process your clinical data to help us provide safe and effective treatment and to protect both parties in the event of legal action. We also process data for legitimate purposes e.g. to send text appointment confirmation or to inform previous patients of new treatments for chronic conditions, billing etc.
Personal data we collect from you may include the following:-
• information that you give us when you become a patient of ours such as name, address, contact details (including email address and phone number).
• details of referrals, quotes and other contact and correspondence we may have had with you
• notes and reports about your health and any treatment and care you have received and/or need.
• patient feedback and treatment outcome information you provide.
• information about complaints and incidents.
• information you give us when you make a payment to us, such as financial or credit card information.
• where you have named someone as your next of kin and provided us with personal data about that individual, it is your responsibility to ensure that that individual is aware of and accepts the terms of this Privacy Policy.

The data that we request from you may include sensitive personal data. This includes information that relates to the mental or physical health or racial or ethnic origin. By providing us with sensitive personal data, you give us your explicit consent to process this sensitive personal data for the purposes set out in this Privacy Notice.
When do we collect personal data about you?
We may collect personal data about you if you:-
• fill in a form or survey for us.
• contact us, for example by email, telephone or social media.
Your personal data will be kept confidential and secure and will, unless you agree otherwise, only be used for the purpose(s) for which it was collected and in accordance with this Privacy Policy, applicable Data Protection Laws, clinical records retention periods and clinical confidentiality guidelines.
Sensitive personal data related to your health will only be disclosed to those involved with your treatment or care, or in accordance with UK laws and guidelines of professional bodies or for clinical audits. We will only use your sensitive personal data for the purposes for which you have given us your explicit consent to use it.
The security of your personal data
We protect all personal data we hold about you by ensuring that we have appropriate measures in place to prevent unauthorised access or unlawful processing of personal data and to prevent personal data being lost, destroyed or damaged.
Any personal data you provide will be held for as long as is necessary having regard to the purpose for which it was collected and in accordance with all applicable UK laws. We have a legal duty to keep clinical records.
All information you provide to us is stored securely.
The transmission of information via the internet cannot be guaranteed as completely secure.
We may occasionally transfer personal information to you via email, or you may choose to transfer information to us via email. Email is not a secure method of information transmission; if you choose to send or receive such information via email, you do so at your own risk.
